If you reside in an area other than San Diego County and are affiliated with an HMO, you may be
asked by your medical group to pursue service with their contracted provider. If your physician
has advised that he would like you to seek your fitting at Kolberg Ocular
Prosthetics', you may ask
for an "out of network referral." In some cases, this may reduce the benefits paid for your
service.
However, if the coordinator of medical benefits at your medical group would like to
arrange rates with Kolberg Ocular Prosthetics, they may contact our office manager Russ Pierce

In most cases, we offer the same rate as their
contracted provider.

We have arrangements with a near by
Holiday Inn for lodging and free pickup and
delivery from the San Diego airport. One day fitting service is also
available with advance scheduling.
